Steps For Submitting A Medical Massage Insurance Claim:
- Contact your insurance provider to see if your active plan covers medical massage therapy. If it does, identify any limitations for coverage and have a copy of this to provide your physician as well as your provider's company. If you have an account with Body Smirks By Ryan Potter, it is required to attach a photo of the portion of your Physician referral/prescription as well as your active qualified plan that shows the details for Massage Therapy.
- Provide a referral or prescription for medical massage from your Doctor stating the goal(s) for massage therapy that your active plan covers and how long the referral/prescription is able to be used.
- Receive a receipt for payment and SOAP notes of your medical massage session.
- Fill out CMS Form 1500 with the appropriate details including the provider's National Provider Identifier registered with NPPES and Massage License Number registered with the Department of Licensing Registration. - Submit a medical massage re-imbursement claim providing your insurance company with everything listed above.
- Please note that standard insurance claims can take about 30 days for re-imbursement. It is strongly recommended to stay in touch with your case manager to make sure that everything has been submitted accordingly for approval.
